Participants will be provided with case management and counseling at no cost while enrolled in this program. They will meet with their Case Managers and all available team members on a monthly basis to develop a  plan based on the needs and goals they want to accomplish. 

 

Case Managers will meet with each participant weekly to assist the family with meeting the needs discussed. Once the participant has completed all of the goals, they will be eligible for transition. During the transition phase, participants will begin to learn all of the skills needed to transition successfully.

Participants will remain with the case manager until they have obtained housing and have shown that it can be maintained. Participants are eligible to stay in the program up until their 21st birthday or their proven success – whichever comes first.
